Emergency financial assistance services are available tocity of Phoenix residentsin crisis situations.Availableservices include:

All emergency assistance appointments will be scheduled using our new appointment phone number for all three Family Services Centers.To schedule an emergency assistanceappointment call602-534-AIDE2433 or 1-866-882-1778.COP is encouraging residents who are eligible to apply before June 30 2019.Income guidelines will be reviewed during intake call. The following documentation should be provided at the appointment will be reviewed on phone call:

• Birth certificate for applicant proof of citizenship or legal immigration status and all children under the age of 18 in the household
• Picture IDs for all adults age 18 and older in the household
• Proof of income for the last 30 days for everyone in the household includes check or pay stubs current award letters from social security SSI SSDI TANF GA UI Workmans comp etc. If terminated letter from employer verifying last day of employment and date last check was received and gross wages
• Social security cards for all members in the household
• Current unaltered lease agreement or mortgage statement eviction notice for rental assistance late notice for mortgage assistance
• Current utility bills if requesting utility assistance
• SRP APS Southwest Gas city of Phoenix Water services water assistance has additional eligibility criteria
